Access to your health records

Accessing your full medical record

Under the Data Protection Act, you can ask us to give you access to, or copies of, some or all of the health records that we hold for you.

Please note that the Data Protection Act allows us to make a charge of between £10 and £50 for the work that your application involves.

If you would like to have this facility, please ask at reception for further information and an application form.

 

Online Access to your medical record

In addition to making appointments and requesting your medication via Patient Access, you can also ask us to give you online access to your medical record.

As of 31 March 2016, many items of "coded" information can be made available to you. These include information such as

  • allergies
  • immunisations
  • medication
  • medical problems
  • laboratory and surgery test results.

If you would like to ask for this information to be made available to you, you will need to complete an application form and show evidence of your identity.

(Please note that, except for lab results, the information available to you does not include any "free text" that the healthcare professional may have written.)
